Urological Cancers (Uro-Oncology)

Surgical Evaluation, Comprehensive Diagnostics, and Oncological Management in Nagpur

Uro-Oncology is the specialized field of urology concerned with the evaluation and surgical management of cancers involving the urinary tract and male reproductive system. The scope of care described on this website should be limited to conditions and procedures specifically established for Dr. Bhole’s practice.

At Ketki Nursing Home & Urology Hospital in Dhantoli, Nagpur, Dr. Sadashiv Bhole provides evaluation and surgical management of selected urological cancers using approaches appropriate to the diagnosis, stage and patient’s clinical condition.

Primary Urological Malignancies

1. Kidney Cancer (Renal Cell Carcinoma – RCC)

Kidney cancer often develops silently without early symptoms and is frequently detected incidentally during routine abdominal ultrasound or CT scans for unrelated issues.

  • Common Symptoms: Painless hematuria (blood in urine), persistent flank pain, a palpable abdominal mass, unexplained weight loss, or persistent fatigue.
  • Diagnostic Staging: Contrast-Enhanced CT (CECT) of the abdomen and pelvis, MRI, and renal function assessments.
  • Surgical Management:
    • Partial Nephrectomy (Nephron-Sparing Surgery): Precise surgical excision of the tumor while preserving the remaining healthy kidney tissue, indicated for localized small renal masses.
    • Radical Nephrectomy: Complete surgical removal of the affected kidney, surrounding fatty tissue, and adjacent lymph nodes when necessary for larger or centrally located tumors.

2. Bladder Cancer (Urothelial Carcinoma)

Bladder cancer primarily originates in the urothelial lining of the urinary bladder. Early diagnostic evaluation is critical to determine whether the tumor is non-muscle invasive or muscle-invasive.

  • Common Symptoms: Sudden, painless gross hematuria (urine appearing pink, bright red, or cola-colored), increased urinary urgency, frequency, or dysuria (pain during urination).
  • Diagnostic Staging: Diagnostic cystoscopy, urine cytology, and CECT Urography or MRI to evaluate the bladder wall and upper urinary tract.
  • Surgical Management:
    • TURBT (Transurethral Resection of Bladder Tumor): An endoscopic procedure passed through the urethra to visually inspect, resect, and pathologically stage bladder lesions.
    • Radical Cystectomy: Complete surgical removal of the bladder, nearby lymph nodes, and adjacent organs in cases of muscle-invasive bladder cancer, followed by urinary diversion (such as an ileal conduit or orthotopic neobladder reconstruction).

3. Prostate Cancer

Prostate cancer can develop without early symptoms and may remain localized for a period of time. Evaluation and treatment depend on the individual clinical situation.

  • Screening & Diagnostics: Elevated Prostate-Specific Antigen (PSA) levels, abnormal Digital Rectal Examination (DRE), multiparametric MRI (mpMRI) of the prostate, and MRI-targeted prostate biopsy.
  • Common Symptoms (Advanced Stages): Difficulty urinating, decreased force in the urine stream, bone pain, pelvic discomfort, or blood in semen.
  • Surgical Management: Radical Prostatectomy: Surgical removal of the prostate and seminal vesicles, with the approach selected according to cancer characteristics and patient factors.

4. Other Urological Malignancies

  • Testicular Cancer: Typically affects younger men, presenting as a painless testicular lump or swelling. Managed via radical inguinal orchiectomy and subsequent staging.
  • Penile Cancer: Lesions or growths on the penis evaluated through biopsy and managed with organ-sparing excision, partial/total penectomy, and inguinal lymph node dissection when indicated.
  • Upper Tract Urothelial Carcinoma (UTUC): Malignancies of the renal pelvis or ureter, treated with radical nephroureterectomy or endoscopic laser ablation in select low-grade cases.

Modern Surgical Approaches in Uro-Oncology

Selected laparoscopic and robotic approaches may support minimally invasive management of appropriate urological cancers, while endoscopic techniques are used for selected bladder and urinary-tract conditions.

  • Laparoscopic & Robotic Surgical Oncology: High-definition 3D/4K magnification allows precise anatomical dissection near critical blood vessels and nerves. Applied in complex radical and partial nephrectomies, radical prostatectomies, and pelvic lymph node dissections.
  • Endoscopic Oncology: Utilization of flexible and rigid endoscopic instruments for diagnostic visualization and transurethral resection (such as TURBT) without external skin incisions.
